PERMISSION TO COLLECT AND RELEASE FROM LIABILITY INSTRUCTIONS
AFTER YOU HAVE CONTACTED ME AND SET UP A VISIT complete a Permission to Collect and Release from Liability form for your visit to Glacier Peak Mines. This form is for either amazonite or topaz.
Print two copies of the Permission to Collect and Release from Liability form and fill out the required information. Keep one copy for your reference.
Scan and attach the completed page to an E-mail and send to glacierpeak@pinnacle5minerals.com. I will acknowledge receipt and retain it in my files for during your visit. If you don't have a scanner, mail the completed form to me or bring it with you. For clubs which have arranged a visit, I recommend you print enough forms and bring the completed forms with you at the time of your visit.
This form signifies you have read the safety briefings and will comply with them. Additionally, it releases me and my company and associates from liability during your visit.
